Stringify Parent Consent and Liability Waiver

About the Program

Stringify is a 501(c)(3) nonprofit organization that provides free after-school violin classes to elementary students. Classes are led by trained middle and high school mentors under the supervision of experienced adult instructors.
Our goal is to make music education accessible to all children in a safe, supportive, and inclusive environment.

1. Participation and Assumption of Risk

I, the undersigned parent or guardian, give permission for my child to participate in the Stringify After-School Violin Program.
I understand that participation in any music or group activity may involve normal physical movements and the handling of instruments, and that while Stringify takes every reasonable safety precaution, some risk of minor injury may still exist.

I acknowledge that my child’s participation is voluntary, and that Stringify, its instructors, volunteers, mentors, and partner schools cannot guarantee a risk-free environment.

2. Release of Liability

In consideration for my child’s participation, I hereby release and hold harmless Stringify, its officers, employees, instructors, mentors, volunteers, agents, and partner schools from any and all claims, damages, or causes of action that may arise from participation in the program, except in cases of gross negligence or intentional misconduct.

This includes any injuries sustained during instruction, rehearsal, transportation, or related program activities.

3. Medical Authorization

In the event of an emergency, I authorize Stringify representatives to seek necessary medical attention for my child and agree to assume full financial responsibility for any resulting medical costs.
I will provide information about any allergies, medical conditions, or special needs at the time of registration.

4. Conduct and Supervision

Students are expected to follow all program rules and respect instructors, mentors, and peers.
All activities will be supervised by adult instructors and trained volunteers. Repeated behavioral issues or unsafe conduct may result in removal from the program.

5. Transportation and Dismissal

Parents/guardians are responsible for ensuring that students are picked up promptly after each session or have permission to walk home.
Stringify is not responsible for students once they leave the designated program area after dismissal.

6. Photo and Media Release (Optional)

By participating in the Stringify Program, I grant permission for my child to be photographed or recorded during classes and performances for educational, promotional, or fundraising purposes.
No personal identifying information will be shared without explicit permission.
If you wish to opt out of this media release, please indicate so on the registration form.

7. Agreement

By submitting the registration form and checking the consent box, I confirm that:

  • I am the parent or legal guardian of the participant.

  • I have read and understood this document.

  • I agree to the terms of participation, release of liability, and emergency authorization outlined above.
